Option 1
To change a name in the registration of an account(s), we ask that the account owner requesting to change their name complete and sign a Name Change Form and submit a certified copy of the legal documentation proving the name change. Click here to complete the request via DocuSign.
Option 2
If the account owner is not able to provide us with the legal documentation, they must sign the form with their new and former name and have both of the signatures guaranteed. To obtain the form, please click here.

The signature guarantee requirement is to protect against unauthorized requests by assuring the required signatures are genuine. A signature guarantee can be obtained at many commercial banks, credit unions, savings associations, or broker-dealers.

A notary public from a commercial bank or brokerage firm is also able to provide an acceptable guarantee. If obtaining a notary stamp, please be sure to include the notary public's business card or a signed letter from the notary public on the financial institution's letterhead.
