Change the registration of my account to a Joint Tenants With Rights of Survivorship (JTWROS) account. To add a joint owner to your account, we will require:
  • A signed letter of instruction or a completed and signed Change of Registration from the current account owner.
  • A completed New Account Application signed by the current account owner and the new account owner.

  • A completed and signed New Account Application is required to verify that all account owners understand the terms of the prospectus and to certify the Social Security numbers of the account owners.
Change the registration of my account to a trust account. To change the registration to a trust account, we will require:
  • A Change of Registration Form completed and signed by all account owners. If an account owner will not be listed as a trustee on the trust account, the signature of the account owner must be guaranteed.
  • A completed and signed New Account Application.
  • A complete copy of the trust agreement.

If you prefer to print and mail the Change of Registration Form and New Account Application to us, along with the required documentation, please click here to obtain the form and application.
  • In place of the trust agreement, we will also accept a notarized Certificate of Trust created by an attorney, which must contain:

    • The name of the trust

    • The name of each original trustee and each successor trustee

    • Any limitations or exceptions in the working of the trust

    • The signature of either a trustee or an attorney
  • Do not submit the request via DocuSign if a signature guarantee is required.
  • The signature guarantee requirement is to protect against unauthorized requests by assuring the required signatures are genuine. A signature guarantee can be obtained at many commercial banks, credit unions, savings associations, or broker-dealers. A notary public is unable to provide an acceptable guarantee.
  • There is an attachment tool within the DocuSign experience for you to provide the required documentation.
