To remove the custodian from the account, we require the custodian to complete and sign a Change of Registration Form. The custodian's signature must be guaranteed on the form.

  • The signature guarantee requirement is to protect against unauthorized requests by assuring the required signatures are genuine. A signature guarantee can be obtained at many commercial banks, credit unions, savings associations, or broker-dealers.
  • A notary public from a commercial bank or brokerage firm is also able to provide an acceptable guarantee. If obtaining a notary stamp, please be sure to include the notary public's business card or a signed letter from the notary public on the financial institution's letterhead.
I would like to remove the Guardian on my IRA since I have reached the age of majority.

We also ask that a certified copy of the former minor's birth certificate be provided.
