Add or Change the Broker for your Account

In order to add or change the broker for your mutual fund account, please complete the form and follow the instructions below.
  • All account owners must sign forms when updating the broker on your account

Add or update my broker on file. Complete a signed Letter of Instruction Form online or fill, sign, and print one here and include:
  • Account Specific Information:

    • Include all names that appear on the account
    • Include the account number or Social Security Number
  • Details including:

    • The Broker’s name, Rep number, name of financial Institution
    • Include a daytime telephone number
  • Your broker’s financial institution must have a selling agreement with the fund in order to be listed on the account. If you are not sure if an agreement exists, please contact your broker.
